feat(fix): add --fix to scaffold missing agent-readiness files

--fix closes the deterministic gaps the scanner finds: it creates any missing
README.md, AGENTS.md, docs/README.md, and CHANGELOG.md from scan facts (package
manager, scripts, entrypoint), then re-scans and reports the new score.

- create-only: never overwrites an existing file
- idempotent: a second run writes nothing
- offline: no LLM, no new runtime deps
- --fix --dry-run previews planned writes without touching disk
- advisory-only findings (package scripts, commit history, entrypoint) stay in the report

New modules: fix.ts (orchestrator), fix-templates.ts (pure renderers),
fix-writer.ts (create-if-absent). Coverage scope extended to the new files.
