feat(codegen): add column_order config option

Add a column_order option (values: "ordinal" | "name") to the shared
helpers.Config, propagated to all three drivers (psql, mysql, sqlite).

When set to "name", generated model columns are sorted alphabetically,
making output deterministic regardless of the order migrations were
applied to the live database. Default ("ordinal") preserves the
previous behaviour for backwards compatibility.
