Integrate canonical streaming across MCP, A2A, and flow

Add shared protocol stream projection, retention, and accumulator helpers.

Migrate MCP notifications, resources, final results, validation, and cancellation cleanup onto canonical stream items.

Migrate A2A task, message, artifact, terminal-state, retention, and cleanup handling onto canonical stream semantics.

Add flow canonical event/session projection with lifecycle, node correlation, cancellation, and UI coalescing support.

Extend provider/tool streaming metadata and protocol e2e coverage.

Add negative tests for malformed and mixed streams, duplicate or late terminals, retention pressure, cleanup failures, and cancellation races.
