Exclude src/PHPStan from coverage

#11 added a PHPStan extension under src/PHPStan/Reflection/. PHPStan
extension classes are exercised by PHPStan during static analysis (and are
covered by the tests/PHPStan integration tests), but they are not run under
PHPUnit's line-coverage instrumentation, so they counted as ~7.6% of
uncovered src and dropped Coveralls to 92.4% even though they are tested.

Exclude ./src/PHPStan from the coverage <source> set so the metric reflects
the code that PHPUnit actually exercises. Tests are unaffected (16 pass).
