add DERIVA_CHATBOT_EXCLUDED_TOOLS to suppress agent-oriented plugin tools from LLM context

  Plugins like deriva-ml-mcp-plugin register tools designed for local-agent use (config file generation, execution lifecycle management, local bag downloads) that are irrelevant or harmful in a browser chatbot context where no local filesystem access exists. Adds DERIVA_CHATBOT_EXCLUDED_TOOLS (comma-separated exact tool names) which permanently removes named tools from the tool list sent to the LLM on every turn. Stopgap until ADR-0002 lands tool-side `applies_to` scoping in deriva-mcp-core. Four tests cover passthrough with no config, named exclusion, no-match passthrough, and whitespace trimming.
