fix(membership): AddOrganizationMember skips PAT all-projects rows

A PAT holding only a pat_granted all-projects policy would hit
ErrAlreadyMember when trying to add a granted org role — that policy
grants project access, not org membership. Filter it out of the
existing-policy check, matching the SetOrganizationMemberRole fix.
