testing(cli): migrate @medplum/cli from Jest to Vitest (#9336)

* Migrate @medplum/cli from Jest to Vitest

Add vite.config.ts with node environment and serial test execution.
Convert tests from jest.* to vi.*, including async vi.importActual for
partial node:fs mocks and default exports on ESM default-import mocks.

Replace aws-sdk-client-mock-jest matchers with commandCalls() assertions
to avoid Chalk compatibility issues with Vitest 4. Exclude test files
from tsc emit. All 191 tests pass.
