thrift/outbound: Support TMultiplexedProtocol

This adds support for making a Thrift client multiplexed. You can simply pass
the `thrift.Multiplexed` option to your client when constructing it to make it
use TMultiplexedProtocol which changes the Thrift envelope name to
`$serviceName:$methodName`.
