refactor: dedupe form-field builders across ConnectForm and overlay

Phase 5 of frontend decomposition (behavior-preserving):
- Add pure helpers to utils/form: fieldsFromHistory, fieldsFromProfile,
  clearedJumpFields (+ tests)
- ConnectForm: handleHistoryClick/handleLoadProfile/handleExtraLoad*/
  clearJumpFields now use the shared builders
- NewConnectionOverlay: fillFromHistory/fillFromProfile/clearJumpFields use
  the shared builders; key file readers use readFileText
- Removes ~5 copies of the large FormFields object literal
