Refactor code to get the TokenManager from the Restlet Context

Make code changes to put the TokenManager instance into the context when
the application starts.  Create OAuthServerResource to provide a
convenience method to get the TokenManager instance from the context.
Refactor the ServerResources to extend OAuthServerResource.
