:sparkles: v0.1.7: sidecar tools in UI, TurnEnd threshold refactor, bug fixes

- AgentsSidebar and StatusBar now display sidecar tools from ResourceStore.get().tools
- Fixed sidecar tools not being injected into active orchestrator (stale session_description guard)
- Hooks.TurnEnd: dispatch is now unconditional; threshold check moved to implementation
- Sidecar.SkillReflector: owns @tool_threshold (5) check in reflect/2
- SidecarManager: handle :stdout/:stderr to prevent FunctionClauseError crash
- SidecarManager: kill stale epmd node registration before spawning sidecar
- ResourceStore: document .tools (sidecar) vs .registered_tools (local) distinction
- Update specs/sidecar.md, hooks.md, and all CHANGELOGs accordingly
