docs(llms): list complete claude defaults in limit-patterns line

The Rate-limit-retry paragraph called out "Default limit patterns" but
showed only `You've hit your limit,You've hit your session limit,
API Error: 529 (claude, with transient 5xx)`. The codex side was fully
enumerated, making the half-enumerated claude side look like the
complete list rather than an abbreviated one. Spell out all entries to
match the source of truth in pkg/config/defaults/config and the README
table.

Addresses Copilot finding on PR #362.
