chore: scaffold release-plz, finalize publish metadata, rename toolkit (#42)

- Add tsoracle-openraft-toolkit publish metadata (readme, documentation,
  keywords, categories, docs.rs config) and pin `version = "0.1.0"` on
  every internal path dep so `cargo publish` accepts the workspace.

- Rename the toolkit crate from `openraft-toolkit` to
  `tsoracle-openraft-toolkit` (directory via `git mv`, package name,
  ~35 import sites, failpoint identifiers, log prefixes, docs.rs URLs).
  No behavior change.

- Scaffold release-plz: `release-plz.toml` with shared workspace
  versioning, changelog updates, git tags, and GitHub releases.
  `.github/workflows/release-plz.yml` runs two jobs: `release-pr`
  opens/updates a release PR on every push to main, and `release`
  publishes when that PR merges. Requires the `CARGO_REGISTRY_TOKEN`
  secret before first run.

- Trim Makefile to drop the manual release-bump/publish/tag targets
  now that release-plz owns the flow; keep `release-dry-run` as the
  local validator. Add tsoracle-openraft-toolkit and
  tsoracle-driver-openraft to RELEASE_CRATES (both publishable now).

- Update CONTRIBUTING.md: refresh the crate table with the two
  openraft crates, drop the cargo-edit prerequisite, replace the
  manual "Release checklist" with the release-plz flow plus a
  one-time bootstrap section.
