Expose remap_hostname() and remap_url() as public plugin API

  Replace the private _remap() + init_hostname_map() pair with two public
  functions delegating to Settings.hostname_map: remap_hostname() for bare
  hostnames and remap_url() for full URLs. Both are exported from
  deriva_mcp_core so plugins can apply the Docker hostname map when
  constructing direct connections to DERIVA-adjacent services (e.g. DerivaML).
  Eliminates the duplicate module-level map copy in context.py.
