feat: integrate godx diagnostics into api_server

- Refactored `SetupLogger` to return the `*slog.Logger` instance.
- Moved `godx` diagnostics initialization into `ApiServer` to ensure
  consistent logger usage.
- Updated `middleware.RequestLogger` to use the injected logger rather
  than the default global instance.
