:sparkles: Allow duplicate agent types — remove type uniqueness constraint

Multiple agents of the same type are now permitted (e.g. two developers
working on different features in parallel). All agents are uniquely
identified by their id; type is display metadata only. list_team + ID
targeting makes type uniqueness unnecessary.

- Removed ensure_type_available/2 from spawn_agent execute_fn
- spawn_agent tool description updated to reflect the new behaviour
- spawn_agent system prompt section updated: "never spawn a type that
  already exists" replaced with guidance to check list_team for reuse
- CHANGELOG, specs/teams.md, docs/guides/teams.md updated
