feat: add MCP stdio transport, GIF demo for Claude Code and Cursor setup

- Add --stdio flag to mcp-server for stdin/stdout JSON-RPC transport
- Refactor RPC processing into transport-independent processRPC()
- Handle notifications/initialized (no-response) per MCP spec
- New mcp-setup.tape and rtmx-mcp-setup.gif showing one-command setup
- README MCP section updated with Claude Code and Cursor setup instructions
- REQ-MCP-006 added with 5 subtests for stdio transport
