fix(playground): use AI SDK transport config for ToolHive internal MCP server

Same `@ai-sdk/mcp@1.0.42` regression as the workload fix, but for the
ToolHive-internal MCP server (`mcp_toolhive`). Split the single
`createToolhiveMcpTransport` helper into two: an AI SDK config builder
(`createToolhiveAiSdkTransportConfig`) used by `getToolhiveMcpInfo` and
`addToolhiveMcpTools`, and a raw SDK transport factory
(`createToolhiveRawMcpTransport`) used by `createRawMcpClientForServer`,
which still goes through `Client.connect()`.
