docs: add CLAUDE.md for future Claude Code sessions

Compact onboarding guide covering only the non-obvious bits future agents
need to be productive: composer scripts, the layered Filter → Controller →
Service → Storage flow, the exact 8-step bypass evaluation order, the BC
facade contract (MaintenanceStorage, IpUtils, Controllers\Maintenance), the
$_GET-after-Services::request() testing pitfall, and the tooling quirks
(deprecation ignores in PHPStan/Psalm/Rector, CodeQL Actions-only scope,
master/main workflow trigger).
