fix: resolve Scrutinizer variable definition warnings (#50)

Initialize variables at the start of their scope to satisfy Scrutinizer's
static analysis in tests/fixtures/mocks.py. These were false positives
(variables were always defined before use), but explicit initialization
improves code clarity and eliminates the warnings.

Changes:
- Initialize device_name/device_id before discovery flow logic
- Move DOMAIN import to top of scope with other imports
- Simplify step_id assignment to single-line conditional

All 456 unit tests still passing.
