:bug: Tool output truncation — 2000 lines or 50 KB, whichever first

Cap tool results before they are stored in session history. Both limits
are always enforced: line truncation is applied first, then byte
truncation on the result. Outputs exceeding either limit are suffixed
with [output truncated].

Also update CHANGELOGs, specs, and guides for execute_fn/3 signature,
identifier+identifier_type targeting, base_url always required, and
session resume fixes introduced in this release.
