namer,integrity: support unnamed codec layout

Add namer.ObjectLocationMissing sentinel that puts NewLayeredNamer
into unnamed mode: the per-codec objectLocation segment is omitted
from every emitted key, so values live at /<name>, hashes at
/hash/<hashLocation>/<name>, and signatures at
/sig/<sigLocation>/<name>. Object names whose first slash segment is
"hash" or "sig" are rejected to avoid collisions with the category
markers.

CodecBuilder no longer substitutes "objects" when WithObjectLocation
is not called; an unbuilt builder is unnamed by default. Existing
tests that asserted the /objects/<name> shape now opt in explicitly
with WithObjectLocation("objects").
