perf(pagination): default page size to 100 (#87)

* perf(pagination): default page size to 100 to match API default

The previous default of 20 was conservative — the Humaans API itself
defaults to 100 (max 250). Bulk reads via Humaans.Pagination.stream/3
now issue 5x fewer requests by default. Callers that need smaller
pages can still pass page_size: 20.

* test(pagination): cover default page size for stream/3
