Skip Pro-only endpoints gracefully and dedup consent issues each tick

Two follow-ups from watching the read-only logs:

1. Free-plan private repos 403 both /rulesets and /branch-protection with
   "Upgrade to GitHub Pro or make this repository public to enable this
   feature." planRulesets already swallowed the rulesets variant; now
   planBranchProtection does the same via a sentinel error from
   fetchExistingBranchProtection. Repos like chrisns/legible2 and
   chrisns/blog stop crashing planRepo and the rest of the plan still
   evaluates.

2. upsertConsentIssue now runs a proactive dedup pass on every entry —
   collect all consent-labelled issues for the repo, close every open
   duplicate above the lowest-numbered survivor, and proceed with that
   canonical issue. The same logic that ran post-create now runs pre-find
   too, so the bot can tidy up loops it caused on previous runs even on
   cron ticks where the changeset is empty.
