ci: use RELEASE_PLEASE_TOKEN PAT for release-please tag pushes (#46)

Switches release-please from the default GITHUB_TOKEN to a fine-grained
PAT stored as RELEASE_PLEASE_TOKEN. GitHub intentionally suppresses
workflow triggers for events caused by GITHUB_TOKEN to prevent
recursive runs, which meant each lib's publish-*.yml workflow did NOT
fire when release-please pushed the release tag. Operators had to
manually `workflow_dispatch` the publish workflow after every release
PR merge (e.g. ngx-numeric-range-form-field@5.1.0).

With a PAT-authored push the tag event is treated as a normal push
and the publish workflow runs automatically. PAT scope: contents R/W,
pull requests R/W, workflows R/W on this repo only.
