docs(plan): invalidation filter on spacesGraph reads, plus operational details

Fixes two correctness bugs from the second deep review: out-of-order
invalidation-before-target would leave the target permanently validated,
and the extraction-graph cleanup claimed for npadef: entries was never
implemented in SPARQL. A single FILTER NOT EXISTS on ?inv npa:invalidates
?np, added to every read from npa:spacesGraph (tier INSERTs, admin-seed
query, attachment validation), fixes both and keeps npa:spacesGraph
purely add-only.

Adds an Operational details subsection covering the structural-DELETE
detection via ASK-then-DELETE, a single materializer mutex for rebuild
serialization, startup cleanup of orphan npass:* graphs, the
npa:currentLoadCounter monotonic-persistence invariant, and polling
semantics for trust-state flip detection.
