docs(plan): add dct:created stamps and explicit tier INSERT sketch

Every extraction entry now carries dct:created copied from the source
nanopub's pubinfo so consumers can sort chronologically without a JOIN.
Tier INSERT scoping is shown concretely with an admin-tier sketch
joining ?entry npa:viaNanopub ?np. ?np npa:hasLoadNumber ?ln and
filtering by the delta bound; npa:hasRootAdmin seed case traverses
npa:hasDefinition to reach the originating nanopub's load number.
