database/clickhouse: fix distributed tests by adding shard/replica macros

ReplicatedMergeTree without an explicit ZooKeeper path uses automatic
path assignment via {uuid}/{shard}, which requires the macros to be
defined in config. Add <macros> with shard and replica values to the
cluster XML so the distributed tests pass on a single-node setup.
