docs(plan): rename to gen:RoleInstantiation, add gen:hasRole extraction, widen admin instantiation

Renames the grant-type nanopub and its materialized entry to
gen:RoleInstantiation. Introduces a separate extraction for gen:hasRole
attachment nanopubs (typed gen:RoleAssignment in npa:spacesGraph, using
<spaceIRI> rather than the space-ref). Reorders subsections to
Space -> hasRole -> SpaceMemberRole -> RoleInstantiation. Emits the
admin role-instantiation entry for any gen:Space nanopub (root or
update), keeping npa:hasRootAdmin narrowed to self-rooted nanopubs.
