feat: report per-batch progress in rag_ingest_datasets background task

  `_run_dataset_enricher` now accepts a progress_callback and fires it after
  each batch of 10 rows, reporting "N/total rows (X%), Y chunks indexed".
  `rag_ingest_datasets` wires this to TaskManager.update_progress via the
  mutable-list-ref pattern so get_task_status returns live progress for
  large datasets (e.g. ~1K rows = 100 progress updates).
