ci: publish Helm chart as an asset of the hub GitHub release (#1209)

* ci: publish Helm chart as asset of the hub GitHub release

Trigger the chart workflow on release:published (filtered to v* tags)
instead of tag push, package the chart locally and upload the .tgz onto
the existing hub release, and use chart-releaser only to refresh the
gh-pages index.yaml with URLs pointing at that release.
