CI: GitHub Actions + Coveralls, README badges

.github/workflows/test.yml runs on push to master, tags, and
pull_request: go vet → go build → go test -race with a
coverage profile → upload via coverallsapp/github-action. Uses
actions/setup-go@v5 on stable Go with module cache.

README gains the standard four-badge row: pkg.go.dev reference,
GitHub Actions test result, Coveralls coverage, Go Report Card.

Coveralls needs the repo enabled at coveralls.io (it'll pick up
GITHUB_TOKEN automatically — no extra secret required).
