refactor(problems): migrate to workspace format and remove legacy api

- Migrated all legacy '.problem.ts' definitions to the new workspace directory
  format ('problem.json', 'files/', 'tests/', 'solution/').
- Removed legacy problem files under 'src/problems/' and 'src/problem-api.ts'.
- Removed migration scripts and commands from 'package.json'.
- Updated problem types in 'src/types.ts' to only support the new format.
- Configured 'tsconfig.json' and 'oxc.config.ts' to ignore fixture contents.
- Updated 'README.md' and 'AGENTS.md' to document the new problem format.
