added Appriasal for multi Rails dependency testing

* Runs tests under all supported versions of Rails (latest)
* Minor test changes to fix issues with Rails 3
* Added Ruby 2.1 to Travis config (except under Rails 5)
* Updated standard test suite to use Rails 5.0.0.1
* Removed init.rb (relic from vendor/plugin days)
