Clean up cached-predictions docs: separate examples, drop comment clutter

README, docs/cached.md, docs/quickstart.md all had the same problem:
a single code block with commented-out alternatives instead of
separate, clean examples per loader.  Replaced with individual
examples, each introduced by a one-line description.  CLI examples
in docs/cached.md split from one big bash block into per-format
snippets with explanatory text between them.

Also fixed the --mhc-cache-file flag-table entry in docs/cached.md —
it said "Requires --mhc-cache-format" which hasn't been true since
auto-sniff landed.
