feat: remap internal hostnames and fix trailing-slash URL handling

  Added hostname_map / ssl_verify support (DERIVA_CHATBOT_HOSTNAME_MAP,
  DERIVA_CHATBOT_SSL_VERIFY) so the chatbot container can reach Credenza
  and the MCP server via their Docker-internal names rather than the public
  hostname. Settings.remap_url() applies the map at call time; all outbound
  Credenza and MCP calls go through it. Mirrors the identical pattern in
  deriva-mcp-core.

  Also injects <base href> from PUBLIC_URL into the index page so relative
  asset URLs resolve correctly whether the browser hits /chatbot or /chatbot/.
