fix: engine island formation bugs and concurrency safety (#112)

- Fix orphaned peers when createIsland fails during flush
- Fix ghost peers when disconnect fires during split/merge await
- Fix crash when islands.get() returns undefined during concurrent disconnect
- Fix geometry dirty flag set after island deleted from map
- Fix mergeIslands parameter shadowing the closure islands Map
- Fix LIVEKIT_ISLAND_SIZE config value ignored (maxIslandSize hardcoded)
- Add null-safe guards in onPeerPositionsUpdate, onPeerDisconnected, split/merge
- Add post-await re-validation in createIsland and mergeIntoIfPossible
- Skip deleted islands in mergeIslands iteration
