Fix generator structure and restore SimpleCov scope (#202)

- Move add_notifiable_to_subscriptions generator back to its own
  directory under lib/generators/activity_notification/ following the
  Rails generator naming convention (<name>/<name>_generator.rb)
- Move migration template to lib/generators/templates/migrations/
  alongside the existing migration.rb template
- Restore SimpleCov filter to /lib/generators/templates/ (was
  incorrectly broadened to /lib/generators/)
- Add generator spec for AddNotifiableToSubscriptionsGenerator
