docs: add status bar icons reference table

Document the ten status bar mode indicators (▼ ◉ ↩ ≋ ⊟ # b ± ✓ ∅)
so users can decode what's shown on the right side of the TUI. Each
row pairs the icon with its toggle key and a one-line meaning, plus
a note about the row's fixed-width graceful-degradation behavior on
narrow terminals.

The icons were previously described only in CLAUDE.md (AI-only
knowledge), leaving end users with no way to look them up. The new
section sits between the View toggles table and the Custom
keybindings section.

Applied to all four user-facing docs that carry the keybindings
reference: README.md, site/docs.html (with a matching TOC entry),
and both plugin usage.md reference copies (.claude-plugin and
plugins/codex, kept byte-identical per project convention).

Bumps the Claude Code plugin version to 0.7.1 (plugin.json and
marketplace.json) because plugin reference files changed.
