Update expression loading design: three flags with implicit join keys

Replace single --expression flag with --gene-expression,
--transcript-expression, --variant-expression. Each bakes in the
join key (gene_id, transcript_id, variant). Simple default: just a
file auto-detects columns for Salmon, Kallisto, RSEM, StringTie,
Cufflinks. Full form: name:file:id_col:val_col.
