Vendor erlang_decimal to avoid OTP application name collision (#3)

The erlang_decimal package registers the OTP application name "decimal",
which collides with Elixir's decimal package. Vendoring the code as
cvss_decimal/cvss_decimal_conv eliminates the separate OTP application
and avoids the conflict. See https://github.com/egobrain/decimal/issues/8
