* refactor: variadic config patterns and *bool fields for optional configs

- healthcheck: New() now uses variadic cfg ...Config with config.Merge()
- metrics: NewMiddleware() uses variadic config, Enabled changed to *bool
- pprof: New() uses variadic config, all Enable* fields changed to *bool
- config: add BoolOrDefault() helper for *bool dereferencing
- server: fix metrics ServerAddr defaulting and path routing bugs
- tests: update all *bool field usages to use config.Bool()
