fix: Allow custom watchers to see excluded dirs

app.Exclude was applied via fsWatcher.Ignore, which suppressed all
events for excluded paths. This prevented custom watchers from reacting
to changes in excluded directories. Move the exclusion check into
the file change handler so custom watchers run before it.
