fix: use pathMatches for exempt paths in rate limit and request logger (#66)

Change exact string comparison to pathMatches() for consistency
with other middleware (BasicAuth, CORS, CSRF, SecurityHeaders).
This allows wildcard patterns to work in exempt path configuration.
