fix(convert): prevent server crash from unhandled exceptions in controllers

- Guard against missing +title= in CRS definition parsing
- Convert convert, get-entrance-stats, get-entrance-time-infos to async/await with try/catch
- Add process-level uncaughtException and unhandledRejection handlers in app.js
- Add CRS fixture without +title= to cover the crash scenario
