refactor: address PR review comments - shared consts, rename, remove immediate poll

- Move provisioning state constants to pkg/consts for codebase-wide reuse
  (SDK Machine.Properties.ProvisioningState is *string with no typed constants)
- Rename agentPoolName to aksMachinesPoolName in poller to match application-layer naming
- Remove immediate first poll from PollUntilDone (redundant with InstantOptions)
- Replace raw string literals with consts.ProvisioningState* across all files
- Add new tests: CreatingThenFailed lifecycle and RetryBudgetResetsOnHealthyState
