Bump Sphinx to 9.1.0 (#15615)

* Bump Sphinx to 9.1.0

We've been using a loose pin on an old version of Sphinx for quite some
time now.  This updates it to a hard pin on a precise version of Sphinx;
our documentation build is no longer just isolated to our repository
where we can fix issues as they arise, so hard-pinning makes it more
reliable for downstream repos like Qiskit/documentation.

It's worth considering using a frozen fully-pinned Python environment
for the documentation build in the future, for the same reason.

* Move `QuantumCircuit.name` documentation to a standard location

Relying on type hints in the `__init__` method requires analysis of the
actual code, whereas doing it in the class body has built-in
language support.  This makes it far easier and more reliable for Sphinx
to pull out the information, but also other typing tooling.

* Move `DAGDepNode` type hints to class level
