Improve Windows application icon

- Switch from bitmap to PNG
- Switch from non-squares to square
- Old: 16x14, 32x28, 48x42, 128x112
- New: 16x16, 32x32, 48x48, 64x64, 128x128, 256x256
- Add two sizes: 64x64 and 256x256
- File sized increased from 24,094B to 60,468B.
- The design is the same.
