Remove Score field from optimizer tool responses (#3971)

Remove the Score field from ToolMatch to avoid exposing internal
distance metrics in tool search results. The hybrid search combines
cosine distance (semantic) and interpolated BM25 scores (keyword),
which are not directly comparable and could mislead consumers.

Score is still computed internally for ranking but is no longer
included in the response. Also cleans up the "relevance score"
wording from the FindTool doc comment.
