Remove slug field support from frontmatter

URLs are now always generated from filenames, not from frontmatter slug field.
This simplifies the system and removes potential confusion about URL sources.

Changes:
- markdown-utils.ts: Changed slug generation to only use getBaseFilename()
- markdown-utils.test.ts: Updated test to verify slug ignores frontmatter
